Early preterm, very low birth weight baby was transferred on day 1 of life with Severe RDS and Pneumothorax from Surendranagar by Team Orange with ICU on wheels, with 2 Intercostal drainage tubes in situ and ventilator support to Orange Neonatal and Pediatric Intensive Care Unit, Chandkheda, Ahmedabad. After admission baby was commenced on HFO ventilation with 100% oxygen, central line secured for TPN, baby required inotropic support for initial days. Gradually baby improved hence shifted to conventional ventilation after 4 days of HFO ventilation, baby had HsPDA with moderate PPHN making more trouble for already compromised lungs, however, the baby fought like a warrior, responded well to medical management, allowed extubated after 7 days to HHHFNC, required rescue doses steroids for oxygen weaning. Things went smooth since then,

now baby is ready to go home after winning a long battle.
